Duties & Responsibilities  :
Perform any other required nursing duties.
Check assignment of patients as well as special assignments.

Receive a full bedside hand over from outgoing nurse. This is to include the following  :
Patient - General condition, ID band, IV cannula, IV Fluids and IV labels skin state, safety measures in place, medication, problems, tests or procedures done and still to be done, invasive devices, wound dressings, drains, catheters.
Documentation – patient file, medication profile, flow sheet, IV labels, special notices (e.g. NPO, fistula) all to be checked.
Environment – tidiness, cleanliness, and maintenance required (.e. call bells not working, lights not working).
Equipment – necessary equipment available in working order correct settings, clean.
Introduce to patients, family and attendees and find out if there are any problems. Ensures that are all familiar with environment and equipment.
Complete physical assessment and document accurate within the first hour of the shift and then according to patient condition and unit protocol.
Perform any blood extractions as needed and fill in all request forms correctly.
Assist with meals.
Prepare and assist with procedures in the unit.
Prepare patients according to policy/protocol for procedures out of the unit.
Give post procedure care according to policy/protocol. Do full physical assessment immediately post procedure.
Accompany patients going for procedures out of the unit.
Assist patients with hygiene and toilet needs.
Administer medications and documents according to protocol/policy.
Give appropriate education to patient and family and refer to any other specialities when necessary (.e. dietician, smoking clinic, etc).
Send all medication prescription to pharmacy promptly and follow-up that these are received back and are correct. Check all unit dose medications and refer any problems to head nurse/in charge nurse.
Admit/transfer/discharge patients as required, ensuring that all necessary documentation is fully completed and that patient belongings and medications are dealt with according to policy/protocol.
Supervise the cleaning and preparing of the bed and room prior to next admission ensuring that it is in accordance with infection control policy.
Follow up results of laboratory tests, referrals, diagnostic tests and document them correctly.
Assist wherever the head nurse/in-charge nurse requests.
Assist with ward clerk duties when he/she is not on duty.
Fill in logbooks when necessary and initiate ova’s when needed.


Inform the in-charge of this.
Inform head nurse/in-charge of any supplies needed, equipment needing repair, general maintenance needed.
Carry out special assignments – crash cart must be checked within first hour of the shift.
The patient assigned nurse is responsible for ensuring that the crash cart is clean and restocked after an arrest.
Keep in charge nurse/head nurse informed at all times about change in patients condition, new/changed orders, procedures, tests done or to be done, results, unusual occurrences.
Give comprehensive bedside handover to incoming nurse.
