1) Patient Adherence To The Therapy :
a) Asthma,COPD,diabetic,obesity,drug abus
e and smoking cessation camps and clinics to the people who are working in OMIFCO(Oman India fertilizer company) & OLNG(Oman liquefied national gas).
b) Prescription error monitoring and assist the doctors for prescribing information regarding medicines from other specialties.
c) Calling to the outside doctors for substituted medicines.
d) Answer to the doctors queries regarding better drug combinations (as I get different patient feed backs), adverse reactions, contraindications, manufacturing companies and also new drug updates.
e) Updating with BNF (British National Formulary), ONF (Oman National Formulary), CIMS, Drug today and C&D monthly pharmacy news updates (UK).
2) Total Pharmaceutical Care On A Regular Basis :
a) Find out what prescribed medicines, herbal and OTC product the patient has been taking.
b) Asking the patient about known allergies.
c) Checking that the medicines are prescribed for its intended use.
d) Checking the prescription to see if the dose is too high or too low.
e) Checking the prescription to see if there is any possible drug – drug, and drug food interactions.
f) Follow-up with patient to see they have side effects from any medicines.
g) Educating the patient or family member on the appropriate use of his/her medicines.
h) Educating the patient on how to self managing their chronic condition through self monitoring and appropriate adjustment of their medicines, diet and exercise.
i) Train the patient on how to use medical devices (e.g. :- glucometer, peakflow meter, BP monitor, spacer, nebulizer or inhaler etc.)
j) Evaluating how well a medicine is working by asking the patient or examining lab test results (if available).
k) Communication with patient doctor regarding any medicine related problems.
l) Referring the patient to the doctor or other professional for a problem that should be assessed by that other professional.
